FHSAA REQUIRED FORMS -- To participate in any Central Catholic Athletic Sport, the Florida High School Athletic Association requires the following General Eligibility forms to be kept on file by the school.
Preparticipation Physical Form (EL2) - This completed form must be kept on file by the school. This form is valid for 365 calendar days.
Consent and Release from Liability (EL3) - This completed form must be kept on file by the school, and it is also good for 365 calendar days from the date of parent signature.

FHSAA REQUIRED COURSES
It is mandatory for all students to complete the FHSAA Concussion and Sudden Cardiac Arrest courses. These courses must be passed prior to a student participates in practices or games. These courses can be taken at home, please click here for directions. Upon completion of the courses, please print a copy of the certificate and bring it to your coach or the Athletic Department prior to your first practice.
Course 1: "Concussion in Sports" - 21 minute course
Course 2: "Sudden Cardiac Arrest" - 41 minute course
